Providing home health care is another alternative to long-term care facility placement. Most seniors and disabled adults need assistance that can be provided in the community without displacement. The community and home provide familiar surrounding and a continuity of family and friend relationships.
Consumer Directed Services is a Medicaid based program providing personal care services, enabling anyone disabled or elderly to live as independently as possible in their own home.
Merit CDS, Inc. can help you to enroll in the Consumer Directed Services (CDS) program. A friend or family member may be hired as an attendant, with the exception of a spouse or anyone under the age of 18. The attendant must have a clean background check through the Missouri State Family Care Safety Registry before being employed by a CDS participant.
Merit CDS, Inc. will train your caregiver to assist you with tasks such as meal preparation, housekeeping, laundry and hygiene assistance. Let us help you hire a friend or family member to assist you with your home care needs.
To Qualify for CDS Services,
a Person Must:

Be receiving or eligible to receive Medicaid benefits

At least 18 years of age

Have a physical disability

Be able and willing to recruit, hire & manage their own care attendants

Seeking to/or living in an independent living situation

Be able to self-direct their care services
What kind of help can I receive at home?
Through a State approved Plan of Care, an individual can have assistance with daily tasks such as:
• Meal Preparation,
• Personal Care,
• Medication Reminders,
• Light Household Tasks,
• Laundry,
• Transportation for Shopping & Errands
Individuals receiving Medicaid benefits will need to meet the Missouri Division of Senior and Disability Services (DSDS) requirements to qualify for caregiver services. DSDS will complete an assessment to determine the individual’s needs and forward a personalized care plan to Merit CDS, Inc. Once a care plan is received, Merit CDS, Inc. will help you hire the appropriate aide to begin your services.
